Medicare Guidelines and Patient Eligibility

Peoples Home Health provides health care solutions where our patients live. Those who require skilled nursing, rehabilitative therapy, assistance with daily activities and are under the care of a physician are eligible for home health services.

The following list includes some of the patients who may be eligible for admission to our home health services:

  • Recent hospitalization
  • Patients with wounds
  • New diagnosis
  • New medication and/or change in medication regimen
  • Creating independence through education and caregiver support training
  • Significant change in condition
  • Patients who are at risk for falling
  • Altered dietary intake
