RN Case Manager - Crestview - Assesses plans, implements, coordinates, monitors and evaluates options and services to meet client’s health needs through appropriate communications and utilization of available resources to promote quality, cost-effective outcomes. Via assessment of client, the RN will develop and implement a Plan of Care based on utilization guidelines and Clinical Pathways.
